Подготовка и протезирование пациентов с низкими клиническими коронками зубов (реферат, курсовая, диплом, контрольная)

Актуальность исследования.

Одним из весьма важных обстоятельств, осложняющих решение клинических задач в ортопедической стоматологии, являются низкие клинические коронки. Несмотря на соблюдение всех требований препарирования, недостаточная площадь культи зуба не гарантирует надежную фиксацию ортопедической конструкции, например, искусственной коронки. При протезировании пациентов с низкими клиническими коронками врач-стоматолог вынужден использовать для усиления ретенции полость зуба и каналы корней [30,53,59,60,68], либо удлинять края искусственной коронки [66,212,252]. Депульпирование существенно снижает биологическую и функциональную ценность зуба [50,54]. Увеличение длины края ортопедической конструкции пагубно влияет на состояние пародонта [80,198,263,316,340,379,395,400]. Клинические и гистологические исследования, проведенные после протезирования, показали, что воспаление десневого края возникает там, где определяется поддесневое расположение края реставрации, независимо от используемого материала [61,228,284]. Однако положение края конструкции относительно гребня альвеолярной кости с точки зрения обеспечения нормального состояния десны является еще более важным фактором [218,272,369].

Многие пациенты с нарушением пропорциональности между высотой и шириной зубов не удовлетворены своим внешним видом, не зная возможностей исправления этого. Устранение таких эстетических дефектов стало важной проблемой для стоматологов [119]. Приемлемым ее решением является специальная хирургическая и ортодонтическая подготовка пациентов с низкими клиническими коронками, после которой усиливается ретенция, исчезает однозначная необходимость депульпирования зубов для улучшения фиксации за счет внутреннего пространства зуба, а также снижается травматическое действие края протеза на десну. К тому же 3 после таких вмешательств появляется возможность диагностики и лечения кариеса корня зуба [19,233,317,403].

Несмотря на очевидные достоинства оперативных и ортодонтических методов коррекции нарушений пропорций зубов и улучшения фиксации ортопедических конструкций, в практической стоматологии они применяются редко. В отечественной литературе мы не нашли публикаций на эту тему, хотя врачи знакомы с ними и иногда используют их разные модификации, среди которых преобладает удаление части прикрепленного эпителия бором во время препарирования зубов, либо обыкновенная гингивэктомия, проводимая хирургом-стоматологом, без соблюдения клинических показаний. Метод удаления части десны бором, во время препарирования зуба, отличается многими недостатками, вызывает стремительную, неконтролируемую рецессию десны и нарушение эстетики. Гингивэктомия, проводимая на какую-то среднюю величину, без учета анатомо-морфологического строения пародонта, также может считаться неприемлемым. Имеются различные методы увеличения низких клинических коронок зубов, однако до сегодняшнего времени детальной и систематической оценки ближайших и отдаленных результатов их применения не проводилось [145,403]. К тому же при этом использовались преимущественно субъективные (клинические) методы исследования[ 149,152].

Цель исследования.

Целью настоящего исследования является совершенствование методов подготовки и протезирования пациентов с низкими клиническими коронками.

Задачи исследования.

1. Обосновать понятие «низкая клиническая коронка зуба» .

2. Изучить способы усиления ретенции искусственных коронок при невозможности хирургического или ортодонтического удлинения клинических коронок опорных зубов.

3. Изучить значение типа искусственной культи и степени охвата твердых тканей зуба искусственной коронкой и его влияние на сопротивление зуба к перелому при замещении полного разрушения коронки зуба различными способами.

4. Оценить возможности известных методов специальной хирургической подготовки зубов по удлинению НКК и предложить способы их совершенствования.

5. Оценить состояние краевого пародонта опорных зубов, прошедших процедуру удлинения их клинических коронок, после протезирования несъемными конструкциями с помощью лазерной доплеровской флоуметрии.

6. Изучить в динамике состояние периодонта опорных зубов после протезирования несъемными конструкциями (искусственными коронками и винирами) при предварительном удлинении их клинических коронок с помощью специальной хирургической подготовки зубов посредством периотестировання.

Научная новизна.

Проведенные исследования позволили получить новую информацию по следующим вопросам:

— обоснование понятия НКК с точки зрения функции и эстетики.

— состояние краевого пародонта опорных зубов после протезирования несъемными конструкциями (искусственными коронками) при предварительном удлинении их клинических коронок с помощью специальной хирургической подготовки зубов. 5.

— прочность фиксации искусственных коронок при различных способах изменения рельефа препарированных зубов.

— определение изменений демпфирующей способности пародонта опорных зубов в результате изменения соотношения внутрии внеальвеолярных их частей при увеличении высоты клинических коронок.

— значение степени охвата твердых тканей опорного зуба искусственной коронкой и его влияние на прочность зуба при замещении полного разрушения коронки зуба различными методами.

Практическая значимость исследования.

1. Путем анализа фотографии лица пациента с обнаженными зубами предложен метод оценки высоты клинической коронка передних зубов.

2. Разработаны алгоритмы проведения увеличения высоты клинических коронок зубов различными методами.

3. В случаях противопоказания хирургического или ортодонтического удлинения клинических коронок опорных зубов предложены наиболее рациональные способы усиления ретенции искусственных коронок.

4. Сравнительная оценка функциональных возможностей литой металлической культевой вкладки и композитной культи на основе стандартных постов позволяет выбрать наиболее прочный вариант в зависимости от степени охвата твердых тканей зуба искусственной коронкой.

Основные положения, выносимые на защиту.

— Понятие НКК должно быть обосновано как с точки зрения функции в качестве опоры искусственной коронки, так и эстетики, определяемой соответствием пропорций зубов и ориентировочных размеров частей лица конкретного человека.

— Минимальный охват искусственной коронкой твердых тканей зуба, наряду с искусственной культей, существенно усиливает прочность несъемной ортопедической конструкции.

— Хирургическое удлинение НКК с использованием методики, сохраняющей контур и структуру десневого края, и последующим протезированием несъемными конструкциями восстанавливает здоровье краевого пародонта после операции при улучшении эстетического эффекта, исходя из индивидуальной нормы пациента.

На Всероссийской научно-практической конференции посвященной проблемам геронтологии в стоматологии г. В. Новгород, 2005 г.

Полученные результаты используются в учебном процессе на кафедре ортопедической стоматологии с курсами имплантологии и эстетической стоматологии ТГМА. Результаты работы внедрены в практику ортопедического отделения стоматологической поликлиники медицинской академии г. Твери и сети СК «Спектра» ООО «НМЦ» г. Москва.

выводы.

1. НКК — это коронка зуба человека, не удовлетворяющая как: (а) эстетическим требованиям пропорциональности лицевых параметров, так (б) функциональным критериям ретенции искусственной коронки. Разработанный способ определения идеальных размеров передней группы зубов позволяет определить наличие НКК, а также величину необходимого удлинения, с целью получения оптимального эстетического результата.

2. Увеличение высоты клинических коронок с 3 до 5 мм существенно усиливает ретенционные свойства протеза. Создание ретенционных пунктов в виде горизонтальных насечек статистически достоверно увеличивает степень фиксации (прочность) протеза. Уменьшение конусности в пределах с 10° до 6° не влияет на увеличение ретенционных свойств.

3. При восстановлении зубов со значительным разрушением их коронковой части большая прочность выявлена у цельнолитых металлических культевых штифтовых вкладок по сравнению с композитными на анкерных штифтах. Дополнительный охват твердых тканей зуба искусственной коронкой существенно увеличивает прочность восстановленного зуба при замещении полного разрушения его коронки различными способами.

4. При проведении хирургического удлинения клинической коронки зуба, иссечение кости альвеолярного гребня (остеоэктомию) предпочтительно осуществлять с помощью ручных инструментов в сравнении с машинными.

5. Сравнительный анализ состояния микроциркуляции пародонта группы ранее протезированных пациентов, имевших поддесневое расположение края протеза, показал, что после проведения удлинения клинической коронки зуба и адекватного расположения несъемной ортопедической конструкции относительно зубодесневого прикрепления, статистически достоверно происходит улучшение состояния микроциркуляции в этой.

154 области и приближение основных показателей микроциркуляции к данным контрольной группы (интактный пародонт).

6. При сравнительном анализе состояния микроциркуляции пародонта ранее не протезированных пациентов установлено, что после проведения удлинения НКК происходит статистически достоверное изменение показателей микроциркуляции, которое, однако, укладывается в пределы границы нормы.

7. Удлинение клинической коронки зуба ведет к увеличению его подвижности из-за изменения соотношения внутрии внеальвеолярных частей зуба, однако это увеличение остается в пределах границы нормы при соблюдении правил проведения удлинения клинической коронки зуба и последующего протезирования.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ.

1. При восстановлении зубов с тотальными и субтотальными дефектами коронковой части в качестве основы для искусственной коронки следует отдавать предпочтение цельнолитым металлическим культевым штифтовым вкладкам, превосходящим по прочности композитные со стандартными анкерными штифтами. При этом охват искусственной коронкой не только искусственной культи, но и двухмиллиметрового (по крайней мере) участка твердых тканей оставшейся культи корня зуба, существенно повышает устойчивость к разрушению (перелому).

2. При отсутствии необходимого объема твердых тканей зуба для надежного восстановления целесообразно проводить увеличение клинической коронки зуба. Способ удлинения НКК зависит от конкретной клинической ситуации.

3. Для профилактики развития хронических локализованных пародонтитов и рецессии десны в области края коронки необходимо перед планированием степени его погружения под десну получить данные о величине зубодесневого прикрепления (БШ) с целью определения её индивидуального размера. В случае малого размера БШ (менее 1,5 мм) погружение края ортопедической конструкции под десну не рекомендуется. При большем значении БШ (2,5 мм и более) возможно минимальное погружение в зубодесневую бороздку с созданием уступа.

4. Для снижения риска развития воспаления краевого пародонта и улучшения прогноза протезирования, необходимо более широкое информирование среди врачей-стоматологов принципа сохранения зубодесневого прикрепления (концепции БШ) при протезировании несъемными ортопедическими конструкциями.

5. Для определения оптимальной высоты клинических коронок зубов целесообразно использовать предложенный метод, основанный на пропорциональности размеров зубов и ширины радужной оболочки глаза человека.

6. Применение разработанных нами и внедренных в практику алгоритмов хирургического и ортодонтического увеличения НКК помогают избежать ошибок при планировании и подготовки к протезированию, проведении ортопедического лечения и улучшить его функциональные и эстетические результаты, а также прогноз.
